I think with the masterbation function when your next to monsters or animals that are with another npc (zombie girl and female student in bathroom) you can join in the fun. I mean these zombies/creatures are distracted so the character can join in by standing in front of it and pressing the masterbate function a new scene will play and once the player is done they can get going.
